Objective: To inform the ophthalmologist of the importance of ruling out the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) in the current context of this pandemic, in a patient with neuro-ophthalmological symptoms without respiratory compromise.
Observation: 41-year-old male presented to the ophthalmologist for palsy of the III and IV cranial nerves and anterior optic neuritis of the left eye. A Polymerase Chain Reaction (PCR) test was requested for SARS-CoV-2 being positive.
Conclusions: Neuro-ophthalmological involvement in a patient with SARS-CoV-2 infection is an alarm data for a severe course of the disease, diagnosing it in the absence of respiratory symptoms is a diagnostic challenge, we must carry out a careful questioning about a recent history of respiratory symptoms or fever, and perform RT-PCR for SARS-CoV-2, avoid delays in its diagnosis, will improve the prognosis of life of the patient.
